Lakeview Health’s Intensive Targeted Case Management provides individualized coordination of care for clients with complex behavioral health and substance use needs. Our case managers conduct comprehensive assessments to identify clinical, social, and environmental needs, then develop service plans that ensure clients receive appropriate interventions and community supports.
Core Functions:
Targeted Case Management improves engagement in treatment, reduces unnecessary hospitalizations, and promotes long-term recovery by addressing both clinical and social determinants of health.
Our Intensive Outpatient Program (IOP) offers a clinically structured level of care for individuals requiring more support than standard outpatient treatment. IOP provides evidence-based therapeutic interventions designed to address substance use disorders, co-occurring behavioral health conditions, and relapse prevention while allowing clients to maintain daily responsibilities.
Program Components:
IOP reduces relapse risk, enhances coping mechanisms, and provides a structured pathway toward recovery without requiring inpatient admission.
Medication Management at Lakeview Health integrates psychiatric evaluation, prescribing, and monitoring to support clients with behavioral health and substance use disorders. Our clinicians utilize evidence-based practices, including medication-assisted treatment (MAT) for substance use disorders, combined with therapeutic support and care coordination.

Peer Support and Recovery Skills
Group therapy provides a supportive, structured environment where clients can share experiences, learn from peers, and practice new skills. Groups are facilitated by licensed clinicians and focus on relapse prevention, emotional regulation, coping skills, and building healthy support systems.
Strengthening Family Relationships
Parent-Child Therapy supports families in improving communication, resolving conflicts, and building stronger bonds. Our clinicians use family systems approaches and evidence-based models to address behavioral challenges, enhance parenting skills, and create a supportive home environment that promotes healing.
EMDR Is Effective for Trauma
EMDR therapy is a highly effective, evidence-based treatment for trauma, PTSD, and related mental health concerns. By engaging bilateral stimulation (eye movements, tapping, or auditory tones), EMDR helps clients process distressing memories, reduce emotional reactivity, and reframe negative beliefs.
